Sign Up Via The Platform & Upload Credentials

Create an account straight away with Better Health HMR using your pharmacist accreditation and contact details. You’ll gain access to a completely no obligation referral service that connects with local GPs and clinics requiring home medication reviews for their patients.

Receive Alerts Via SMS or Emails When There Is A Referral In Your Area

GPs click a button on the PMS to initiate an HMR and our referral service matches the request with a local accredited HMR pharmacist via SMS or Email alert.

Accept Referral Via The Better Health HMR Platform

The HMR pharmacist chooses to accept referrals they can perform through the Better Health HMR platform.

Conduct HMR & Upload Review

The home medicine review pharmacist conducts their review using our online software with direct access to advice from the GP or other specialist right here on the platform. When your interview is complete, you simply submit the HMR from the platform and it’s returned directly to the referring GP electronically.

Easily claim via the PPA Portal

Once the credentialed pharmacist uploads the review to the GP, they can lodge the claim for the HMR via the PPA Portal. A small fee is payable to Better Health HMR for each request that you accept.

Simply contact us via email or phone, or alternatively fill out the form and we will get in contact with you to join our team.

To conduct HMRs (also known as DMMRs), you must be a Credentialled Pharmacist. We do require you to upload proof of your credentials when you register for an account with Better Health HMR.

When you set up your Better Health HMR pharmacist account, you can select your location and the areas you are happy to travel to and conduct HMRs (DMMRs). When a GP posts a referral in these areas, you will be instantly notified via text message and/or email, depending on your preferences.

The number of HMRs (DMMRs) you can expect to conduct will depend on your area and your availability. Better Health HMR has the largest GP clinic network in Australia for HMR/DMMR referrals and we bring on new clinics regularly. 

The Pharmacy Programs Administrator (PPA) limits the number of HMR services per calendar month to 30 per Credentialled Pharmacist. For further information, visit: https://www.ppaonline.com.au/programs/medication-management-programs/home-medicines-review

HMR initial services and follow-up services must be claimed via the PPA Portal by the end of the next calendar month after the initial Interview or follow-up Interview was conducted (e.g. Interviews undertaken in March must be claimed by 30 April).
